Incident Report - Student
Consult the student’s Permission to Administer Medication Form if necessary. Refer to the student’s behaviour chart and the AGS Behaviour Discipline Policy if the incident involved social & physical interactions between
students and individuals. 

Employee Intervention - Second Step Written Reminder - This form is used in instances where a staff member/employee does not meet the desired expectations, behaviours, or does not respect rules/code of conduct. This is a written reminder of the intervention that took place during a meeting with the employee. Following this, the Manager must explain the rest of the disciplinary process in case the employee does not follow through with the chosen solutions. Please note that this document will be kept in the employee’s personal file.

Application for the permission to administer medication
Application that Parent/Guardian gives permission to authorized staff/teacher of Alaqsite’w Gitpu School to administer medication to their student. For non-prescribed medication and prescribed medication. Specifying method of use, where on the body location to administer, quantity, times and days allowed, with dates start and end, as well as side effects to watch for.
